You may be hosting an event where you are in need of a parent or guardian to sign a waiver in order for their child to attend. Our actions feature allows you to show and require the parent or guardian to sign a waiver only when purchasing a ticket for a child under the age of 18 (or whatever age you want to require it for)

Here is how you can set this up.

Edit Page and Add a Date of Birth Field

From the Pages screen, click the Pencil icon of the page you'd like to capture a parent consent

Under the ticket level you'd like to capture date of birth on, click Add a Question for... button

Select the Date of Birth field

Click the Done button

Now it will ask for date of birth for each person who selects that ticket level

Add Terms Field

Hover over a field below the ticket block and click the plus button to add a field

Select Terms from the Data Collection column

Click into the Label field and update the field label to be whatever you'd like - For example "Parent or Guardian Consent"

Enter the terms you'd like the parent or guardian to agree too in the Agreement Text box

If you'd like for them to provide a digital signature, you can enable "Collect a finger signature" On mobile they'll sign with their finger and on a computer they'll sign with their mouse

7. Click the Done button

Add Name Field

Hover your mouse over the newly added Terms field and click the Plus button to add an additional field

Select Name field

You can update the Name label to be whatever you'd like- For example Parent or Guardian Name then click the Done button

Add a Text Field to Capture Relationship

You can click the Plus button to add an additional field

Select Text Field

Update the label to be whatever you'd like - For example Relationship to Minor

Click the Done button

Click the Save Now button

Create the Action

Click the Actions tab

Click Create Your First Action or the plus button to add an additional action

Click the Select Trigger dropdown

Select Date of Birth

Select Age Less Than - Any date of birth under the age you set, it will trigger the Parent/Guardian agreement to show

On the right column click the Select Action dropdown and choose Show

Then select the Terms, Name, Relationship fields you had created

Click the Save Action button

Click the Publish button to set your changes live

You can click the Preview button to test this out. You can try setting a date of birth higher or lower than the age you set to see how it will show or hide the parent consent fields.
